CLINICAL HISTORY- Low back pain.

Axial and sagittal T1 and T2 weighted sequences were utilized.

At L5/S1, there is degenerative disc disease with annular bulging of the disc margin and a broad-based focal protrusion in the midline and extending to the right impinging upon the anterior aspect of the thecal sac as well as the right nerve root. This protrusion is consistent with a hroad-based herniation.

At L4/5, there is mild degenerative disc disease and mild annular bulging. No focal herniations are evident.

L1/2, 2/3, and 3/4 appear well maintained in height and signal without bulging or herniation.

The conus is normal in appearance. There is no abnormal signal arising from the osaeous structures. There is no stenosis.

IMPRESSION: Degenerative disc disease at L5/S1 with annular bulging and a broad-based protrusion in the midline and extending to the right consistent with a herniation. This directly impinges upon the phecal sac and the nerve root as it exits toward the right neural foramina.

Mild annular bulging at L4/5,
